In the case of trips abroad of Italian minors under the age of 14 (not accompanied by at least one of the two parents or by the person acting on their behalf), it is necessary to request a mention on the Italian passport or an accompanying declaration.
The following documents shall be attached to the declaration:
a copy of the identity document that the child will use for the journey;
copy of the identity documents of the parents (or guardians);
a copy of the identity documents of the accompanying persons;
booking or travel ticket (with indication of the itinerary).
The declaration may be made:
in person in the Embassy, by appointment for “consular services” to be booked through the system Prenotami
The Embassy can however request, in specific cases, the physical presence of the operators of parental authority.
Parents or holders of parental responsibility must submit a declaration to the Authority responsible for issuing the passport (clicc qui to download the form accompanying declaration minors), which is normally the one in whose constituency the applicant resides (Questura or Consulate), in which they indicate the name of the person/s/s or the body in which the child is entrusted, requiring the entry to be made on the child’s passport or in a separate certificate.
The declaration is normally valid for one trip (to and/or return) outside the country of residence of the child under 14, with a specific destination. The maximum period of validity of the declaration (within which the date of departure and re-entry must be included) is 6 months. However, the validity may not exceed the expiry date of the child’s passport.
In the case of journeys that involve the crossing of different States, it is up to the parents or those who take their place – based on the modalities of the journey itself – to indicate only the country of final destination or all the individual countries visited.
In the case of entrustment to an entity or transport company it is not possible to mention the passport and will be issued only the attestation.
Any costs of sending the certificate or passport to the applicant shall be borne by the applicant.
